Medicare will work in Guam. Guam is about 80% populated by US military and affiliates. It is really pretty isolated, so be aware of that. It is also not all that cheap (surprisingly). They don't really encourage a lot of expat immigration because it is a pretty small place with relatively small infrastructure.

Mexico still has deed restrictions within 20 miles of coast, so there is risk. Then again, Thailand also has the same issues for foreigners. There are ways of getting around it by forming a company but it is a hassle

Salzburg is fantastic and very welcoming to TOURISTS, but Austria is not the most open or tolerant society (still a great place...). S'pore is an awesome place to work, but not so much to retire. As you said, extremely expensive, though health care and amenites are outstanding. Thailand is good, but the health care is questionable outside Bangkok. Better to go to KL, which has excellent healthcare and is just as inexpensive and welcoming to foreigne...more
