Actually, For me the doctor's charges in most cases are not as bad as the hospital charges. Hospitals overcharges are outrageous. I went to two docto'rs for some moles. I think you hit the nail on the head. The states where people actually work for a living are doing much better then the welfare states. Interesting as these numbers are, things change VERY fast. Extremely poor customer service and poor streaming make NETFLIX  a No-Go for me. I get streaming from overseas much faster and clearer then NFLX. I don't see how they survive.
